00 If you are a positive and personable individual looking for a satisfying and fun opportunity to make a real difference in the lives of people with intellectual and developmental disabilities, join our team at RHA Health Services! This position provide basic case management services to medically fragile children who are at risk of institutionalization and whose health, safety, and well-being can be maintained in his/her primary residence.
Responsibilities include:
Establish eligibility for services, obtain client histories, perform initial assessments, develop a plan to provide basic services, explain available services and programs, and refer clients to appropriate programs/agencies for other needed services.
Evaluate and document client programs Conduct interviews and work closely with clients, families, other relatives, school, medical providers, and other community agencies contacts to obtain information to be incorporated into a social history which will aid in planning for required services as appropriate to the individual plan of care.
Provide basic case management services as the lead member of a Social Worker/RN interdisciplinary team.
